Office Policies At Plaza Park Dermatology

APPOINTMENTS

At Plaza Park Dermatology, we value your time and understand that our patients expect to be seen at their scheduled time. For this reason, we do not overbook the schedule and if you arrive late, may be rescheduled. We ask that you arrive 15 minutes prior to your appointment to complete the registration process. If you need to reschedule or cancel, please call at least one full business day before your appointment (Monday appointments should be changed by Friday). A $50 fee will be applied to appointments changed with less than 24 hours notice.

Plaza Park Dermatology is contracted with the following health insurance plans:

  • Empire Blue Cross/Blue Shield
  • Medicare
  • Multiplan/PHCS
  • United Healthcare
  • 1199 SEIU

Please call 718.522.6647 to verify in-network plans. We can also discuss using out-of-network benefits or self-pay option if your insurance carrier is not currently listed.

If a referral is required by your health insurance plan, please contact your primary care provider to obtain one prior to your visit. Your appointment may be rescheduled if you do not have a current referral at the time of your visit.

A valid insurance card, photo ID and credit card are required to complete in-office registration. Please bring these items to your appointment.
